FRAIL is an evidence-based decision-making and assessment tool that uses Censeo Inc.'s Fall Manager software to aid the caregiver in reducing incidences of falls through proper assessment, prevention, and documentation. To calculate individual resident risk for falls, Fall Manager's proprietary assessment tools consider resident demographics, physical attributes, co-morbid conditions, and medical history as well as current conditions such as mobility, stability, medication use, visual acuity, equipment use, ADL abilities, etc. The resident's risk level is then determined, allowing the caregiver to implement the appropriate individual preventive care plan.

When falls do occur, Fall Manager uses standardized terminology to document each incident, such as location, time, resident's physical and mental state, injuries acquired and the resident's activities immediately preceding the fall. The caregiver is then able to take note of the resident's past history regarding falls (including resources allocated such as walkers or other assistive devices) and revise the resident's care plan in an effort to reduce future occurrences.
