The WRAP Program: Wound Risk, Assessment & Prevention
Censeo's WRAP program utilizes our Wound Manager software to help heal wounds and to reduce pressure ulcer development through proper assessment and care. Wound Manager is an evidence-based assessment and decision-making tool designed to calculate individual resident risk for wound development. Censeo's proprietary assessment tools evaluate resident demographics, physical attributes, co-morbid conditions, medical history and current condition. The resident's risk level is then determined, allowing the caregiver to apply the most appropriate individual preventive and therapeutic care plan. As a further advantage, the caregiver is given the option of using either of the Norton or Braden assessment scales (which are included along with Wound Manager's own risk assessment tool).
As with all of Censeo's software solutions, a unique and valuable feature of Censeo's Wound Manager is its use of wireless handheld technology, which allows the caregiver to document and review resident records at the point of care. Wound Manager uses standardized terminology to record location, dimensions, appearance and characteristics of each wound then tracks the care and healing process. It also documents the resources used (such as dressings, topical agents, specialty surfaces, etc.) over the course of the treatment to enable the facility to assess and improve the efficiency of their care procedures.
Research and Development
During the research that led to the development of Wound Manager, an Experimental Group consisting of 1500 patients were treated using risk-directed ulcer management techniques over the course of their hospital stay. By the end of the trials the incidences of pressure ulcers for patients in the Experimental Group had dropped to below half that of the Control Group. Additionally, specialty surface utilization rates were substantially lower. (See figures 2 and 3.)
Research and experience have confirmed that the use of evidence-based assessment tools, such as Censeo's Wound Manager, lead to improved outcomes and lower costs.
